No Dues Form for Resignation
Build department-specific No Dues questionnaires in the Dynamic Form Builder and wire them directly into your resignation workflow approvals.

- A clear list of clearance items each department needs (e.g., IT assets, finance settle-up, knowledge transfer)

Creating a No Dues Questionnaire
Three steps to publish a department-specific clearance form.

Add a new template
Click Add New, provide the Template Name, associate the form with the target department, and click Create.

Drag in your fields
Use the attributes panel on the right to drag and drop the relevant data types into the questionnaire. Match the field types to what each department actually needs to capture.

Save & publish
Once the layout is complete, click Save. The new questionnaire will now be selectable inside the Resignation workflow approval stages.

Field Reference
Each field type captures a distinct kind of response. Pick the type that matches the information you need.

| Checkbox | Lets approvers tick one or more boxes — ideal for confirming a list of items handed over (laptop, ID card, access cards). | Optional |
| Radio button | Single-choice selection — good for Yes/No or status-style answers (e.g., Outstanding dues: Yes / No). | Optional |

Best Practices
Design Tip
Design each questionnaire with the receiving department in mind — IT clearance needs different fields than Finance clearance. Once saved, a form is selectable as a standalone stage inside the Resignation workflow, so keep each one focused on a single department's scope.
Frequently Asked Questions
Can I use the same No Dues form across multiple departments?
Technically yes, but we recommend a form per department — it keeps the questions relevant to each approver and keeps the audit trail clean.
What happens to a No Dues form after an employee exits?
The submitted form stays attached to that employee's resignation record and is visible from the Approval Flow view in the Admin panel.
Can I edit a form after it's been used in a workflow?
Yes — edits apply to future resignations. Previously submitted forms keep the version of fields that existed at the time of submission.
Does the employee see the No Dues form?
No. No Dues forms are filled in by approvers (IT, Finance, HR, etc.) during the clearance stages of the resignation workflow.
